Free Vehicle Drop-Off & Repair Authorization Form Generator

Fill in a drop-off and repair authorization at the counter or in the customer's driveway, then print it or save it as a PDF for signatures. Shop info and your standard terms are saved on this device; nothing is uploaded.

Have a lawyer review your terms. This is a form template, not legal advice. Rules for written estimates, authorizations, storage fees and liens vary by state and locality, and this tool doesn't check them. Edit the terms to match what your attorney approves.

How to use

  1. Enter your shop info once (it's shared with the estimate and inspection tools on this device).
  2. Edit the storage terms and standard terms to match your policies, and have a lawyer review them. They're saved for every future form.
  3. At drop-off, fill in the customer, vehicle, concern, authorized amount and call-before-exceeding limit, and walk around the vehicle to note existing damage.
  4. Print or save as PDF and get the customer's signature. Keep a signed copy with the RO.

FAQ

Is this form legally binding?

This tool makes a printable form template. It isn't legal advice, and it doesn't check your state's rules. Requirements for written estimates, authorizations, storage fees and liens vary by location. Have a lawyer review your terms.

What is a call-before-exceeding amount?

It's the dollar limit the customer authorizes. Above it, the shop contacts the customer for approval before continuing. Record any additional approval (amount, who, when, how) on the form.

Why note existing damage?

A quick walk-around at drop-off documents scratches, dents, glass chips and warning lights that were already there, which helps avoid disputes later. Photos are a good addition.

Are my terms saved?

Yes. Storage terms and your standard terms are saved in this browser, along with your shop info, so they're ready for the next customer. Customer details stay on this device too.

Can I use it for mobile jobs?

Yes. Fill it in on a phone or tablet and print or save the PDF. You can also have the customer sign a printed copy.
